We look forward to seeing you at Key Dermatology. To help your appointment run smoothly and make the most of your time with our dermatology team, please review the information below before arriving for your visit.

Before Your Appointment
To ensure an efficient visit, please complete the following before arriving:

Complete your patient forms through the patient portal if possible by providing the staff with your email. If you do not want to register for the portal, you can download the fillable PDF (click here) and bring it to your appointment, as it does not transfer over electronically.

Bring your insurance card and photo ID

Bring a list of your medications (or photos of the bottles)

Arrive 15 minutes early to allow time for check-in

If you were referred by another doctor, bring any relevant medical records
Completing these steps in advance helps us keep your appointment on schedule and allows your dermatologist to focus on your care.
Preparing for a Skin Exam
If you are coming in for a full-body skin exam, please follow these guidelines:

Avoid wearing makeup, nail polish, or heavy lotions

Remove nail polish or artificial nails if possible

Wear loose, comfortable clothing

Be prepared to change into a medical gown

Your dermatologist will examine your skin from head to toe to evaluate moles, growths, rashes, or other skin concerns.
Early detection is one of the most important steps in preventing and treating skin cancer.
If You Are Being Seen for a Specific Skin Concern
To help your dermatologist diagnose your condition quickly:

Make a list of your symptoms

Note when the problem started

Bring photos if the condition comes and goes

Write down any treatments or creams you have already tried
This information helps us better understand your skin condition and recommend the most effective treatment.
What to Bring to Your Dermatology Appointment
Please bring the following items with you:

Method of payment for copays or balances
Collecting this information ahead of time allows us to process your visit efficiently.
Appointment Policies

We kindly ask patients to provide at least 24 hour notice if they need to cancel or reschedule an appointment.

Missed appointments or late cancellations will be subject to a $50 no-show fee, as this reserved time could have been used to care for another patient. Thank you for understanding and working with us.
